In order to assist you with your investment in your dental health, you may select from the following payment options:
1. Cash and Money Orders
2. Personal Check
This requires personal identification, and a copy of your driver’s license is also required.
3. Credit/Debit Cards
We accept Visa, MasterCard, American Express and Discover.
Visa or MasterCard debit cards are accepted also. Debit card payment allows you easy record keeping and helps you avoid check writing.
4. Credit Care Pre-Authorization
By using this easy-pay method, you authorize us to place a fixed amount on your credit card each month.
5. Insurance Assignment
We understand the value of insurance benefits and will assist you in obtaining your maximum allowance. We will process your claim and estimate your deductible and co-payment amount. Your co-payment is due at the time services are rendered and may be paid by any of the above methods. The final payment made by the insurance company may be different from the original estimate and any overage will be refunded. Any underpayment will be billed directly to you. The treatment choices made are between the doctor and the patient and may not coincide with allowances by the patient or the patient’s employer’s insurance contract. In that event, you are responsible for the entire amount.
Financing
Our office offers a third party financing by
Capital One Healthcare Finance, which requires an application and credit check. This option allows patients to make extend a payment plan, and make comfortable monthly payments according to their budget. To apply for financing please call: Capital One 1-877-559-5050 or

Initial Visit
Full payment is expected for all treatment completed or diagnosed at the first visit. A fifty-percent deposit is requested for treatment requiring multiple visits. This allows you a 90-day interest-free time period in which to pay the additional fifty-percent. All fees are to be paid-in-full when treatment is completed. Any balances that extend past 90-days will incur 2% monthly finance charges.
We will be happy to work with you to plan the most appropriate arrangements for your budget. Financing your treatment will allow you to begin your treatment immediately and will spread the costs over a period of time. To avoid misunderstandings regarding financial arrangements, please read the following information:
Patients with Dental Insurance:
We wish our patients to know that all professional services rendered are charged directly to the patient and that patients are personally responsible for payment of fees. We will prepare the necessary forms or reports to help you obtain benefits from your insurance companies. We do not render our services on the basis that insurance companies will pay all the fees on your account.
Non-Insured Patients:
We wish non-insured patients to know that patients are responsible for full payment as services are rendered. If necessary, we will make financial options for you prior to your appointment. Patients may make payments with cash, check, credit cards and debit cards. MasterCard, Visa, Discover and American Express are accepted. We also offer Capital One Financing, which requires credit approval and allows you to pay on a monthly basis directly to the agency.
Overdue Accounts:
In the event that payments are not received in full within 30-days of services rendered, there will be a repeat billing charge of $2.00 or 2.0% (whichever is greater) of the balance. Patients are responsible for all costs of collection, including, but not limited to, reasonable attorney’s fees. Un-kept appointments after confirmation will be charged a broken appointment fee.
